I certify that I am aware of AIM’s ’No Refund’ policy and that prepaid payments will be used toward a rescheduled appointment time.

Upon receipt of a request to cancel a scheduled appointment, AIM will provide a credit code for the initial session requested. I will be responsible for completing the Schedule an Appointment form to reschedule my appointment and must enter the provided code to receive the credit.

I read, understand and agree to AIM’s Terms and Conditions and would like to proceed with canceling my scheduled session.
